All participants 18+ using the Hellyer County Park Velodrome are REQUIRED to complete and sign a COMBINED Waiver which includes:
- a Santa Clara County waiver
- a Northern California Velodrome Association waiver
- a Rider code of conduct waiver
- a COVID-19 waiver
IMPORTANT!
Waivers must be filled out prior to riding the track. Waivers are valid through the end of the calendar year. Each rider is required to fill out a waiver prior to riding the track if it is their first time at the track that year.

Minors (17 and under)
- Each Minor must have a parent or legal guardian sign a Junior COMBINED Waiver (which includes the COVID, County, NCVA, and Code of Conduct) forms.
- Approved Minors are marked in BOLD in the Completed Signed Waiver List.
- If a group of Minors (3+) would like to attend the track, the group’s organizer must contact info@hellyervelodrome.com for a streamlined group authorization procedure.
